Pueblo Community Health Center is the only healthcare provider in Pueblo County with a clinic dedicated to serving the homeless population. PCHC defines homeless very simply as lacking permanent housing. The individuals and families we serve at our homeless clinic may be found staying in homeless shelters, sleeping on the street or in a vehicle, in a transitional housing program or moving from place to place.

An individual may also be considered homeless if they are unable to maintain their housing situation and are forced to stay with a series of friends and/or extended family members. Individuals who are released from a prison or a hospital may be considered homeless if they do not have a stable housing situation to which they can return.

Pueblo Community Health Center is proud to offer health care services to the homeless people of Pueblo. Our Health Care for the Homeless program is located at 1008 N. Grand Ave, Pueblo, CO 81003. We are a proud member of the National Healthcare for the Homeless Council.
