Human Service Agency is an SD PATH STATE provider.


PATH is an acronym for Project Aid in Transition to Homelessness.


The PATH program funds service people with severe mental illness who live homeless.


PATH providers are local public or non-profit organizations that have received PATH funding from the U.S. government to provide services to people who are not currently connected to traditional services.


Some PATH programs offer services, but are not fully inclusive for all programs (some PATH providers offer all these services, while others focus on offering one or two of them.

Community mental health


Drug abuse treatment


Referrals to primary health care, professional training, housing, educational assistance


Accommodation services as specified in Section 522(b)(10)
