Homeless individuals/people living on the street
Individuals staying in a homeless shelter
Please bring a referral from the shelter to receive medical, dental or vision care
Individuals in a doubling-up situation (staying with a friend or family member)
Please bring proof of residence (utility/cable/phone bill)
